Why Complaints Are Filed

All licensed respiratory care practitioners and all health care institutions must, and any person may, report to the Board a Complaint if an individual licensed by the Board has engaged in act that may be grounds for disciplinary action and/or may not be able to safely and skillfully practice respiratory care. There are many different types of complaints.

Pursuant to A.R.S. § 32-3208 Health Professionals are Obligated to report any involvement in criminal activity to the Board within 10 Days. If the licensee does not comply with this requirement it will be deemed "Unprofessional Conduct" which may lead to further disciplinary action by the Board. How to File a Complaint

There are two ways to file a complaint with the Board, please choose one of the following options:

1. Submit Complaint Online

The online portal is temporarily unavailable while we transition to our new licensing system. Please submit a complaint in writing during this transition period. 

 

2. Submit a Complaint in Writing.
