Application Licensure

Apply for a New License Online  (RRT Credential required)   

To apply for a new license, visit the APPLICATION PORTAL to create an account.


Please note the following:

Once you have completed and submitted your application; Board staff will send you instructions about your fingerprint background check.   If you reside in Arizona you will be utilizing the Arizona Department of Public Safety’s “Field Print” system.

If you reside out-of-state you will be receiving a blue Federal Bureau of Investigation Fingerprint Card (Form FD-258)  For Out-of-State Applicants; it is very important that you read and follow the instructions for completion and submittal of the fingerprint card! [Chain of Custody]  If you do not follow the instructions; your fingerprint card will be rejected, you will be required to resubmit and your license will be delayed.

Applications can generally take 6-8 to process for approval.  When your application is complete (meaning only that we have received all the required documentation) you will receive an email notifying you that you application has been approved and issued.


Applications will not be considered complete without the following:

  • A photocopy of a diploma awarded upon your successful completion of a respiratory therapy training program.
  • A document providing proof of citizenship. A list of acceptable documents can be found in the Evidence of Citizenship document.
  • If you answer "yes" to any of the background questions on the application form, you MUST provide complete written explanation of your answer(s) and all documents related to your answer(s).

If you are, or have ever been licensed in another state, you must request a verification of license(s), completed, signed and authenticated by seal or notarization by the board of every state in which you held a certification or license, or registration as a Respiratory Care practitioner to be sent directly to the Board in Arizona.

  • IF you have not practiced respiratory care or held a license in this or any other state during the three years immediately preceding the date of filing an application for licensure you must complete and submit 20 approved CEUs



When you apply for a initial License Application online, total fees charged are $273.00 (under Board Rule R4-45-102)​

For paper applications, please include a certified check or money order in the amount of $270.00.
