Submitting a license renewal online will provide immediate verification of payment information. This does not mean your license is renewed. All renewal applications must be reviewed. Under the laws and rules, at R4-45-216.

We will continue to process license applications, renewals and other services during this transition period. To submit your request, please complete and submit the appropriate PDF form and make your payment online if required. Instructions for submitting the forms are provided for each Form type.
