The Respiratory Board has taken steps toward environmental sustainability and operational efficiency by transitioning to a paperless office. Please utilize the Online Portal for new applications, renewals, licensee service requests, complaint submissions and licensee look-up.
New Applicants: Visit the Application Portal to submit an application for Licensure.
Existing Licensees: Visit the Licensee Portal to submit your renewal, update your personal information, order a license verification, or download a copy of your wallet card and license.

The following Board Actions do not appear on the Board's website pursuant to Arizona Revised Statutes § 32-3214:
- Disciplinary or non-disciplinary actions older than 5 years
- Complaints resulting in a non-disciplinary letter of concern
- Dismissed complaints
Pursuant to A.R.S. 32-3214(C), a person may obtain public records relating to any licensee, including dismissed complaints, letters of concerns, and disciplinary/non-disciplinary actions older than five years, by submitting a public records request. Pending complaints and investigations may not be disclosed to the public.
